    This is a survey regarding “The Evaluation of the Performance of Student Affairs.” We need your support and help very much.

    In order to provide better service, the Office of Student Affairs wants to understand your needs and the current user experience in the hope of better understanding our offer of service and as a reference of future work enhancement.

    This is a voluntary survey. Every opinion you provide will contribute significantly to future work enhancement and planning of student affairs. The information you provide will only be used for researching. Data users will not know your identity. Please feel free to complete the survey. We will analyze the information according to students’ responses and will announce the result.

    This survey has two parts. First is personal information, then the acknowledgement, the frequency, the satisfaction and the needs of the service of every division of the Student Affairs, including the user experiences, satisfaction, personal points of view, and the needs of the Student Affairs service.

Raffle Activity 　

※Everyday Lucky Prize — During the period of time（10/13-11/2）the system will randomly draw a person who has completely fill out the survey as a lucky prize winner with Ching-Yeh restaurant voucher 500 NTD.

※Power-Developed Prize — scooter dual dashcam、fitness wristbands、wireless Bluetooth earphones。

※Super Great Surprise — In appreciation for everyone’s cooperation, we will draw at least20persons (based on how many people have completed the survey) Every prize winner will get a great value of the Kuang-Fu Restaurant(Family Mart) voucher 500NTD.
(※According to the Office of Finance Cashier Division, the prizes should be included in annual income taxes.)

How to join

Simply complete the survey in time and you can join the “Lottery Drawing Game”!(Everyone is limited to get one prize.) Additionally, you may also join everyday lucky raffle. The earlier you complete the survey, the more chances you get. If you have a bad luck and failed to obtain these big prizes of great value, it’s okay~ There’s still a lucky partaking prize for you!


How to draw the prize

Prize winners in accordance with the regulations of the activity are randomly drawn by the computer system from the data.

How the prizes are sent

Prizes will be sent in two weeks after the event (during the business hours)



Receival and Abstention

Prize winners should bring the student ID cards (or any identification) to the Office of President of Student Affairs(3F, Yun-Ping Building, Kuang-Fu Campus)and claim the prize in person within at weekdays’ office hours. Others cannot claim the prize for you and failing to claim the prize in time will be regarded as abstention.



Notice

1. The notice is clearly written in the activity webpage. Participants accordingly agree with the regulations of the notice at the same time when partaking this activity. Any violations against the rule will result in disqualification and the division’s rights will be reserved to any misbehaviors.

2. The prize winners should go to the assigned place to obtain the prize after the announcement of their name list in time. Failing to finish in time will be regarded as abstention.

3. Participants should ensure the written information to be true and correct, and do not falsely use others’ information. Any violations will result in disqualification. Our division is not responsible for the person, therefore, cannot receive the notifications of the prize winning. Moreover, if there are any violations to others’ right due to the participant, the participant should be fully responsible for it.

4. If there are any problems with the registration caused by computers, the Internet, phone calls, or technical issues, our division is not responsible for it. And the participants should not therefore disagree with it.

5. The prizes announced on the website will be served. If there’s any inevitable problems, our division’s rights of changing the prizes of the same value are still reserved.

6. The prizes of the activity must not be exchanged, transferred, or converted into cash.

7. If there are any special inevitable reasons stopping from the activity ongoing, our division has the rights to cancel, cease, edit, or pause the event.

8. Prizes should be included in the annual income taxes.
